Responsibilities
Tasks
- Prepare reports or case histories
- Appraise clients' needs or eligibility for specific services
- Plan rehabilitation programs with offenders
- Assess client's relevant skill strengths and development needs
- Conduct follow-up assessments
- Develop, co-ordinate and implement the delivery of specific services within the community
- Housekeeping activities
- Food preparation
- Supervise activities of clients living in group homes and half-way houses
- Implement substance abuse treatment programs
- Implement behaviour management programs
- Liaise with other social services agencies and health care providers involved with clients
- Assist in evaluating the effectiveness of treatment programs
- Assist clients/guests with special needs
- Accompany clients to appointments and social outings
- Interacting with individuals, their families/advocates, co-workers and the community respectfully
- Establishing and maintaining effective working relationships with individuals
- Providing support and education in social interactions, communicating, healthcare, coping, community awareness, etc.
- Facilitating community inclusion
- Liaising with the support team, community services, professionals and others
- Preparing documentation objectively
- Working as part of a team in case management
- Participate in the development and maintenance of recreational and social activities
- Attend regular team meetings

- Non-violent Crisis Intervention (NVCI) Certificate
- First Aid Certificate
