The Ridge Meadows Division of Family Practice (RMDFP) is a non-profit society that has been incorporated since 2010 serving the Maple Ridge and Pitt Meadows areas. RMDFP provides approximately 122 members with a strong voice to ensure that our physician members are at the forefront of positive change in our health care system for patients in Ridge Meadows.

Attachment & Evaluation Coordinator is an exciting new position at RMDFP, supporting our new initiative that will transform health care in our community! The ideal candidate will have top-notch Attachment & Evaluation knowledge along with strong data and document management skills.  Support RMDFP and its Members by preparing and managing reports, performing monthly and annual Evaluation reports!

Type of position: Full time, contract

Responsibilities:

  • Support PCN Managers and Partners to develop an attachment tracking system that aligns with Ministry of Health’s registry system and local PCN needs (find a doc, doc to doc, direct Wellness Centre attachment, community attachment)
  • Support the collection and analysis of attachment data
  • Maintain and keep attachment tracking system updated
  • Support the two PCN attachment tracking needs along with codes
  • Support the communications and marketing of the attachment system
  • Work with patients and partners to implement attachment with developed strategy (i.e. one-to-one support to match unattached patients seeking a primary care provider and other referral pathways)
  • Support the development of attachment reports to communicate progress made in reducing attachment gap/target outcomes
  • Participate in working groups and leadership meetings as required
  • Work with recruitment coordinator on physician supply knowledge
  • Other related duties
  • Support PCN Managers and Partners to develop an evaluation system that aligns with Ministry of Health’s needs and local PCN needs
  • Maintain and keep system updated
  • Support the collection and analysis of data
  • Support the two PCN evaluation tracking needs
  • Work with patients and partners to implement attachment with developed strategy
  • Provide and produce monthly, or as needed evaluation reports and analysis 
  • Participate in working groups and leadership meetings as required
  • Support the collection of data from all partners and providers
  • Support the change management of all partners and providers including the attachment codes per PCN
  • Other related duties

Required Skills, Qualifications and Experience:

  • 3 + years in Evaluation in a fast-paced environment
  • Post-secondary diploma/certificate
  • Strong understanding of Evaluation & Attachment
  • Exceptional time management skills with the ability to work in a deadline-oriented environment
  • Proficiency with computers including Microsoft Office Suite
  • Excellent customer service and communication skills with the dynamic ability to engage busy members
  • Strong verbal and written communication skills
  • Familiarity with non-profit sector is an asset

Pay: $23 - $26/hr based on skills & experience
